Competition: Aedile Report Banner/Headers

Specifics

Basically I need a banner and headers for when I want to write a report. Now since I am probably the worst person around with graphics I am leaving it up to you to create these for me. Only trick is you might want to ask me a few questions as to what I would like to see on the report banner or not.
